Part A: Property Waiver: "I grant permission for AOPA to show any aircraft as outlined below." I the undersigned for valuable consideration, the receipt of which is hereby acknowledged, on behalf of my self, my legal representatives, heirs, successors and assigns, grant to Aircraft Owners and Pilots Association and its related and/or affiliated entities, including but not limited to AOPA Membership Publications, Inc. (collectively "AOPA") their successors and assigns the right to use and publish for editorial, trade, advertising and any other purposes, photographic portraits, motion picture material, videos of me, or photographic likenesses or pictures of me or in which I may be included (the "Photos" or "Videos") and to copyright such Photos or Videos. These Photos or Videos may be used in (including, but not limited to) marketing promotions, promotional DVD's, AOPA's website, AOPA's Project Pilot campaign, and other electronic newsletters. I may be included in the Photos or Videos in whole or in part, in composite or distorted form, in conjunction with my name, or reproductions thereof, in color or otherwise, made in any manner and/or medium. I waive any right that I may have to inspect or approve the finished Photo or Video or the advertising or other copy that may be used in connection therewith or the use to which it may be applied. I release and discharge AOPA, their successors and assigns and all persons acting under their permission or authority from any liability or claims relating to such Photos or Videos. Part B: Model Waiver: "I grant permission to show myself or any other persons as outlined below." I the undersigned, being the legal owner of or having the right to permit the taking and use of photographs or motion pictures of certain property for valuable consideration, the receipt of which is hereby acknowledged, on behalf of my self, my legal representatives, heirs, successors and assigns, grant to Aircraft Owners and Pilots Association and its related and/or affiliated entities, including but not limited to AOPA Membership Publications, Inc. (collectively "AOPA") their successors and assigns the right to use and publish for editorial, trade, advertising and any other purposes, photographic portraits or photographic likenesses or pictures of the Subject or in which the Subject may be included (the "Photos" or "Videos") and to copyright such Photos or Videos. The Photos or Videos may be in any, or multiple, mediums and may have printed matter published in conjunction with them. These Photos or Videos may be used in (including, but not limited to) marketing promotions, promotional DVD's, AOPA's website, AOPA's Project Pilot campaign, and other electronic newsletters. I waive any right that I may have to inspect or approve the finished Photo or Video or the advertising or other copy that may be used in connection therewith or the use to which it may be applied. I agree to release, hold harmless and discharge AOPA successors and assigns and all persons acting under their permission or authority from any liability or claims relating to such Photos or Videos, for any reason including but not limited to liability by virtue of any blurring, distortion, alteration, optical illusion, or use. If signing as an agent of a business, partnership or corporation, I warrant that I am fully authorized to do so. |
